How to use the guardian portal (Guardians)

Once the guardian has been invited to the portal they will receive an email that allows them to click a link and register.

Please remember the information you register with as you will need it every time you need to log in.

Registering your account

Once you click the link provided in the email you will be sent to a registration portal. Please fill in the information to sign up. If you have already registered in the past, you can login with the same credentials you previously used.

After you sign up, you will be able to login. If the system doesn't recognize your relation, you will be prompted to confirm the patient's name and last four of the social.

Once that information is provided, ZenCharts will bring you into the document list where you can sign documentation and view previously completed forms.
