How to Document CIWA, COWS, CINA Scales (v3)

Entering CIWA/COWS/CINA Scores

1. Login to ZenCharts and click into the desired client's chart

2. From within the clients chart, navigate to the menu drop down and select medical

3. Click on the Vital Charts sub-menu, and select 'Scale Assessments'

4. Click on the plus sign on the upper right corner of the screen (+) and click on 'Add Scale'

5. In the modal that opens. modify the date/time as needed, and select the scale you wish to complete from the drop down

6. Complete the assessment by selecting the appropriate score for each assessment from the drop down

7. Click 'Save Assessment'

Note: If your facility staff is completing the  CIWA/COWS/CINA assessments within the scale assessments of ZenCharts, each of these scores will all be graphed out for you over time to see how a client's assessment scores are trending overall.
